Position Title: Registered Nurse (RN) / Licensed Practical Nurse (LPN)

Location: Regency Park Nursing and Rehab Center, Carroll, Iowa

Job Duties and Responsibilities:

  • Patient Care: Administer medications and treatments as ordered by physicians, and monitor patient health status to ensure the effectiveness of care plans.
  • Assessment and Monitoring: Conduct patient assessments, including physical and psychological evaluations, and monitor progress towards health goals.
  • Care Planning: Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans for residents.
  • Documentation: Maintain accurate and up-to-date patient records, including health status, treatments administered, and responses to care.
  • Patient Advocacy: Communicate effectively with residents and their families, addressing concerns and providing education about health management and treatment plans.
  • Emergency Response: Respond promptly to medical emergencies, providing appropriate interventions and notifying emergency personnel as needed.
  • Compliance: Ensure adherence to all regulatory and organizational policies, including infection control practices and safety protocols.
  • Team Collaboration: Work as part of a multidisciplinary team to ensure coordinated and holistic care for residents.
  • Education: Educate patients, families, and fellow staff on various aspects of health care and wellness.

Qualifications:

  • Licensure:
    • For RNs: Valid and unrestricted Registered Nurse (RN) license in the state of Iowa.
    • For LPNs: Valid and unrestricted Licensed Practical Nurse (LPN) license in the state of Iowa.
  • Education:
    • For RNs: Associate’s Degree in Nursing (ADN) or Bachelor’s Degree in Nursing (BSN) from an accredited nursing program.
    • For LPNs: Diploma or Associate’s Degree in Practical Nursing from an accredited program.
  • Experience: Previous experience in a nursing role, especially in a long-term care or rehabilitation setting, is preferred but not required.
  • Skills: Strong clinical skills, effective communication abilities, and proficiency with electronic health record (EHR) systems.
  • Certifications: Current Basic Life Support (BLS) certification. Advanced certifications (such as ACLS for RNs) may be required based on job responsibilities.
